KNOW ALL BY THESE PRESENT:
That the undersigned policyowner and member of Pharmacists Mutual Insurance Company appoints the Secretary of Pharmacists Mutual Insurance Company, as Attorney-in-Fact and grants to the Secretary a vote on any matter which may be put before the membership. This proxy shall be effective on the date signed and shall continue, unless revoked by me. This proxy shall continue in full force and effect for no longer than thirteen months from the date signed. My appearance and voting at any meeting of Pharmacists Mutual Insurance Company shall constitute a revocation of this proxy. I further appoint the Secretary with the power of substitution to substitute Aaron Pearce, CEO & President of Pharmacists Mutual Insurance Company, to vote in his stead or absence. This proxy shall be valid for all purposes for which I may vote my membership, including the annual meeting to be held at the home office at 808 Highway 18 West, Algona, Iowa at 9:00 a.m. CDT, Friday, May 17, 2024.
