CONSENT TO RECEIVE ELECTRONIC DISCLOSURES AND NOTICES
TERMS AND CONDITIONS
Pharmacists Mutual Insurance Company is pleased to offer electronic delivery (Paperless Delivery) of notices and documents. You will be able to select policies to be enrolled into this service. Please read the Terms and Conditions carefully which contain important information about your legal rights.
Terminology
In these Terms and Conditions, the words "you" and "your" mean named insured. The words "Pharmacists Mutual", "we", "us" and "our" mean Pharmacists Mutual Insurance Company and its subsidiaries.
Accepting these Terms and Conditions
By accepting to enroll in Paperless Delivery, you agree to these Terms and Conditions and consent to receive important policy documents in electronic format. If you do not agree with any of these terms, you may not enroll in paperless services.
Only the named insured will have authorization to enroll, select policies, and withdraw consent. We will only deliver new document notifications to the named insured. You can log in to the member portal to view, download, and print. It is your responsibility to log in to view your documents. Electronic documents have the same legal effect as printed ones. We may provide you with all communications in electronic format, and may discontinue sending paper communications to you.
We may continue to send paper copies of certain documents if required by federal or state law. Electronic delivery is not currently available for all documents or states.
Scope of Communications to Be Provided in Electronic Format
By enrolling in Paperless Delivery and consenting to electronic delivery, you accept and agree to the following communications received but not limited to:
Insurance policies, schedules, and endorsements
Notices pertaining to scope of and limitations on coverage
Disclosures and notices regarding the use of credit information and consumer reports
Privacy policies and notices
Insurance cards or other evidence of coverage
Any other documents necessary or appropriate to the business relationship between you and us
Hardware and Software Requirement
Documents will be made available in PDF format. To access, you must have a valid:
Email account, with sufficient space and spam-blocking software that does not block our emails
Access to an internet browser

Printer if you wish to print documents
It is your responsibility to maintain a valid email address and immediately notify us of any change in it. If an email is returned to us as undeliverable, we will resend the email. If it is returned a second time as undeliverable, we will presume that you have withdrawn your consent. You will be unenrolled in electronic delivery and all available policy related documents will be sent to you via the United States Postal Service (USPS) to your postal address. Note you will have the ability to access www.phmic.com to view your documents on demand and re-enroll. Documents will be archived for five years in the library.
You may update your email address at any time by clicking the settings option. You will receive a verification email that it was entered correctly and you will then need to follow the instructions in that email. You can also contact Member Services at 1.800.247.5930 ext. 4050 or emailing member.services@phmic.com.
Should we make any changes to hardware or software requirements for Paperless Delivery we will inform you of those requirements. You will be requested to review the revised Terms and Conditions and your continued enrollment will serve as your consent to continue participating in the Paperless Delivery option according to the new requirements.
Request Paper Copies
You may request a paper copy of any document at no charge by contacting us.
Withdraw Consent/Termination
You may withdraw your consent at any time at www.phmic.com. Log into My Account, select Paperless Delivery Preference box, and click on the selection boxes to unselect the policy(ies) you want to remove from paperless policy documents. You may also withdraw your consent by contacting us. All subsequent insurance documents will be mailed to you via USPS. Please allow up to 24 hours to process your un-enrollment. Revocation of your consent shall not affect the legal enforceability of documents provided to you before the effective date of un-enrollment.
If, after electing to withdraw your consent to receive documents electronically, you decide that you want to once again enroll in Paperless Delivery, you may do so by going to www.phmic.com and completing the enrollment process.
Your consent to enrollment in Paperless Delivery will remain in effect for each selected policy until termination or cancellation of the policy, or until you revoke your consent. Paperless Delivery will remain in effect following any policy modification or renewal.
Pharmacists Mutual may amend these Terms and Conditions at any time. Your continued participation in Paperless Delivery will constitute your acceptance of any revisions to these Terms and Conditions. Terms of Use can be found on www.phmic.com when you log into your account under Paperless Delivery.
We are not required to deliver information electronically and may discontinue electronic delivery in whole or in part at any time.
IMPORTANT STATE SPECIFIC DISCLOSURES AND CONSENT LANGUAGE
The following specific disclosures and consent language are required by certain state law. Please read the disclosures or consent language below applicable to the state for which your policy is issued.
GEORGIA:
For policyholders of the state of Georgia who have elected to receive all mailings and communications electronically, you hereby agree as follows: I AGREE TO RECEIVE ALL MAILINGS AND COMMUNICATIONS ELECTRONICALLY. SUCH ELECTRONIC MAILING OR COMMUNICATIONS MAY EVEN INCLUDE CANCELLATION OR NONRENEWAL NOTICES.
TENNESSEE and KENTUCKY:
The policyholder who elects to allow for selected policy documents, available notices, and communications to be sent to the electronic mail address provided by the policyholder should be aware that the election operates as consent by the policyholder for notices to be sent electronically, which may include notice of nonrenewal and cancellation. Therefore, the policyholder should be diligent in updating the electronic mail address provided to the insurer in the event that the address should change.
MISSOURI RESIDENTS:
Missouri law requires that you be specifically advised that this Disclosure and Consent applies to Renewal Offers under Missouri Statutes §379.118. No further state-specific notices are provided herein.
Consent:
By selecting the Paperless option, you affirmatively consent and agree to allow us to provide electronic communications to you as described herein, and express your intent to electronically sign this Disclosure and Consent, including any State Specific Notices applicable to your state. You agree that you have the authority and ability to receive electronic communications on behalf of all insureds under your policy. You also agree that electronic signatures may be used in your dealings with us, that you possess or have regular access to one or more devices which satisfy the hardware and software requirements specified above and that you have provided us with a current and valid email address to which we may send electronic communications to you.
By consenting to receive communications electronically and use electronic signatures, you agree that it is your responsibility to access, open, and read all communications and documents that we send to you electronically. Electronic communications and transactions are legally binding in the same manner as paper communications or transactions.
